Antibiotic Prophylaxis in Surgery

1. What are the principles of antibiotic propylaxis in surgery?   antibiotics

  1. Prophylaxis should be given prior to incision, this is to acheive effective plasma and tissue concentration at time of incision.
  2. Most antibiotics whould be give no earlier that 60 minutes before incision.
  3. They should be given again if the procedure takes longer than 4 hours.
  4. The should be given again in the setting of massive blood loss.
  5. The duration of antibiotics should not be longer than 24 hours.

2. What antibiotics are typically given?   antibiotics

  1. Cefazolin 2g IV for skin
  2. Metronidazole 500mg for GIT
  3. Plus Vancomycin if high risk for MRSA
  4. Replace cefazolin for gentamicin if high risk penecillin allergy
